The California Secretary of State maintains an Advance Health Care Directive Registry. The cost for filing a registration form is only $10. If you are admitted to a hospital, your Health Care Directive can be accessed by your health care provider and your appointed agents can be contacted to make decisions for you.

Every unmarried person should have their Health Care Directive registered. The form can be completed at www.sos.ca.gov/ahcdr/forms.htm.
